In an effort to give you the best possible service, please arrive 20 minutes before your scheduled appointment and bring your ID, insurance card, credit/debit card, and FULLY COMPLETED PAPERWORK.

A $100 deposit is required when making your first appointment — this will be refunded or applied to your visit.

Late arrival or lack of completed paperwork will result in the need to reschedule you and charge a $100 cancellation fee.

Dr. Thunder needs your entire appointment to fully assess your situation.

Please bring the following to your first visit:

  1. Driver’s License/Photo ID
  2. Health Insurance Card(s)
  3. Credit/Debit Card for payment through Credit Card on File
  4. Completed forms from your new patient packet
  5. Reports of any radiographic imaging (x-rays, CTs, and/or MRIs) you have had in relation to your current problem(s)
  6. Any other pertinent medical records (e.g. doctor visits, surgeries, hospitalizations)

  • Due to the nature of our treatment methods, our doctor and therapists spend much of their
    time in close proximity to their patients. Showering and freshly laundered clothing are much
    appreciated.
  • We are unable to provide childcare. Please plan accordingly for children requiring supervision. Dr. Thunder needs your full attention during your visit. You will need to reschedule your appointment and pay a cancellation fee if children are unattended.
  • We require a $100 deposit when making your first appointment which will be either refunded or applied to your visit. However, we charge a $100 cancellation fee if you fail to cancel your first visit appointment. We’re happy to waive this if you call 48 hours in advance of your appointment. This fee is ineligible for insurance coverage and solely the patient’s responsibility.
  • You must arrive with your ID, your insurance card, credit/debit card, and ALL of your paperwork COMPLETED or you will be rescheduled and charged the $100 cancellation fee. Dr. Thunder generally spends an hour and a half with his new patients, and without this time, he will be unable to fully assess your situation.
